Splenic Artery Embolization as a bridge to Splenectomy in severe Transfusion - Refractory Thrombocytopenia Secondary to Myelodysplastic Syndrome

چکیده
Transfusion-refractory thrombocytopenia is an important clinical challenge in the treatment of patients with myelodysplastic syndrome. Hypersplenism can play a major role in splenic platelet sequestration and transfusion refractoriness, and though rare in myelodysplastic syndrome, it can commonly be seen in a wide range of other benign and malignant conditions. Splenectomy has been an effective approach in management of hypersplenism-induced cytopenias, however those who are unresponsive to platelet transfusions are not surgical candidates making this an unfeasible treatment option. There has been increasing interest in the therapeutic value of the less invasive procedure of splenic artery embolization. Here we report the first case of a patient with myelodysplastic syndrome complicated by hypersplenism and severe transfusion-refractory thrombocytopenia in which a splenic artery embolization was successfully used as a bridge to splenectomy, which resulted in platelet transfusion responsiveness and correction of her thrombocytopenia. Reversal of severe thrombocytopenia has multiple important therapeutic values including allowing patients to safely continue receiving myelosuppressive chemotherapy treatments which are sometimes delayed or dose-adjusted due to cytopenias, safely undergo surgical interventions and also open the door for candidacy for stem cell transplantation. In conclusion, splenic artery embolization with or without subsequent splenectomy is highly recommended as a safe therapeutic option in patients with hematologic malignancies. *Corresponding author: Ashkan Emadi, M.D., Ph.D, Associate Professor of Medicine, Pharmacology and Experimental Therapeutics, Director, Hematology and Medical Oncology Fellowship Program, University of Maryland, School of Medicine, Marlene and Stewart Greenebaum Comprehensive Cancer Center, 22 South Greene Street, Room N9E24, Baltimore, Maryland 21201, USA, Tel: 410-328-2596, Fax: 410-328-6896; E-mail: [email protected] Received Date: September 30, 2016 Accepted Date: October 28, 2016 Published Date: November 01, 2016

Partial Splenic Embolization as a Bridge to Total Knee Replacement for a Patient with Severe Thrombocytopenia due to Cirrhosis and Splenic Sequestration
Medical splenectomy by embolization was originally used to attenuate bleeding from varices in a man with cirrhosis and portal hypertension. Despite the procedure being described over 40 years ago with remarkable improvement in its safety profile and clinical outcomes since, it is still used with variable frequency because of concerns that the risk is high and the results are transient. We prese...
متن کاملPartial splenic embolization for refractory thrombocytopenia
When the platelet count falls below 20×10(9)/L, the risk of spontaneous life threatening hemorrhage is concerning for both physician and patient. When medical management fails, splenectomy is often used to manage the severe thrombocytopenia before spontaneous, life-threatening gastrointestinal or intracranial bleeding occurs. We present the non surgical management of such a patient with refract...
متن کاملComparative Study of Patients that Underwent Splenectomy for Immune Thrombocytopenic Purpura With and Without Splenic Artery Embolization
Basis and Objectives: Platelets and/or red blood cells transfusion usually are performed in patients with Immune Thrombocytopenic Purpura (ITP) who are submitted to splenectomy. The purpose of this study is to test if preoperative embolization of splenic artery is able to prevent platelets and/or red blood cells transfusion. Patients and Methods: All patients submitted to splenectomy for purpur...
متن کاملSplenic Artery Embolization for the Treatment of Gastric Variceal Bleeding Secondary to Splenic Vein Thrombosis Complicated by Necrotizing Pancreatitis: Report of a Case
Splenic vein thrombosis is a relatively common finding in pancreatitis. Gastric variceal bleeding is a life-threatening complication of splenic vein thrombosis, resulting from increased blood flow to short gastric vein. Traditionally, splenectomy is considered the treatment of choice. However, surgery in necrotizing pancreatitis is dangerous, because of severe inflammation, adhesion, and bleedi...

Background: Splenic infarction (SI) is a rare clinical entity seldom encountered in veterinary medicine. Its most frequent causes include thromboembolic status, splenomegaly, and cardiac disease. Although thrombotic elements from the circulation provide the most common context for thromboembolic SIs, immune-mediated hemolytic anemia (IMHA) has not been reported as an underlying...
